Exam Procedures for Students
The first step in establishing exam accommodations is to meet with a Disability Service Coordinator. They will provide you with an accommodation form, which is required for further support.
Upon receiving your accommodation form, you will be asked to provide the Disability Services Assistant and/or Learning Success Coordinator with a Student Schedule, Course Outlines (for each registered course in which you have been provided accommodations). You will also be required to provide contact information such as an email address or phone number where you can be reached.
Once these initial steps have been completed:
- Inform the Disability Services Assistant and/or Learning Success Coordinator of any quizzes, midterms, or final exams, the same day an instructor announces them in class (especially those that were not listed on the proposed course outline).
- If an instructor reschedules an exam to an earlier or later date than proposed on the course outline, notify the Disability Services Assistant and/or Learning Success Coordinator in person or by phone or email as soon as possible. If the DS Assistant and/or Learning Success Coordinator is not notified immediately there is a strong possibility that the rescheduled exam may not be written with the DS Assistant and/or Learning Success Coordinator, as her/his exam schedule may be fully booked. In which case, the only other option would be that you would have to write in class without the accommodation.
- Once an exam is scheduled (especially if you have confirmed you will be writing it), and you are unable to make the exam for any reason or you are going to be late, call the DS Assistant and/or Learning Success Coordinator as soon as possible. If there is no answer, leave a message on voicemail explaining the situation.
- Inform the Disability Services Assistant and/or Learning Success Coordinator of any course changes, i.e. course/s changed to audit, dropped course/s, added course/s, or a change in instructor/s of current course/s.
