Plan for tomorrow so you can live for today—a good motto for life! Planning ahead is one of the greatest gifts you can give yourself and your loved ones. This course explores advanced care planning as a vital part of overall life planning—focusing specifically on your health care preferences and personal values.
In the first three sessions, we’ll cover the what, why, who, and how of creating a health care plan that reflects your goals and wishes. You’ll learn how to identify what matters most to you, how to communicate those values, and how to have meaningful conversations with family, friends, and health care providers. We’ll also walk through the essential documentation needed to ensure your choices are respected.
In the final session, we’ll expand the conversation to include Power of Attorney and Estate Planning, rounding out a thoughtful, comprehensive approach to planning for quality of life—right through to the end of life.
Meet the Trainers
Jane DiGiacomo, executive director of Nelson & District Hospice Society. She has received psycho-social palliative care training with Victoria Hospice and BC Hospice Palliative Care Association, navigation training with UBC, doula training with Soul Passages and advance care planning with NIDUS. Prior to Jane's work with NDHS, she was a practicing attorney in New York and was active in not-for-profit management. She enjoys skiing and hiking with her family, along with gardening and renovating her heritage home in Nelson.
Kathy Hrabluk, a retired education administrator and remains a teacher at heart. She and her husband moved to the Kootenays in 2017 and promptly joined LIR, both proving to be great decisions! Kathy stays busy volunteering for Nelson & District Hospice, Nelson Community Food Centre, Columbia Basin Alliance for Literacy and LIR. She loves the beauty of the Kootenays and enjoys kayaking, camping, hiking and cross-country skiing.
